Ted Allen Memorial Award Nomination Form
Deadline for nominations is July 1st
The Ted Allen Memorial Award is presented annually to the CHPA member who has performed exemplary service for the state or club during the three previous years. Those eligible for nomination could fit into into the following categories:
Outstanding tournament director
state officer
club organizer or supervisor
assisting the CHPA or club with special efforts for the good of the sport
